To save time and prepare your therapist for your visit all new clients are required to fill out the intake form at the time of scheduling.
If possible, please do not wear makeup, earrings, necklaces or strong perfumes to your facial appointment.
No children under the age of 12 will be permitted without prior approval.
STRICT AND ENFORCED 48 HOUR CANCELLATION POLICY!
Diverse Skin Solutions is committed to providing you with exceptional service. When a client cancels without giving enough notice, they prevent another client from being seen. Please give at least 48 hours notice prior to your scheduled appointment of any changes or cancellations. You may also reschedule/cancel using the link from the confirmation email. If 48 hours notification is not given, the card on file that is used to secure your appointment will be charged the full Pre-discount cost of the missed/scheduled appointment.
Although a Credit or Debit card is required to secure your appointment, Diverse Skin Solutions & Acne Care, and staff does not see credit/debit card data such as numbers, expiration date or CVV code. All credit card information is encrypted collected using Stripe whose Parent Company is PayPal. ALL credit card information is 100% safe and is not visible to anyone.
As a courtesy, You will receive an email or text reminding you of your service prior to your appointment date. Please understand that it is your responsibility to remember your appointment dates and times to avoid late arrivals, missed appointments, and the cancellation fee.
By scheduling the appointment you are authorizing Diverse Skin Solutions & Acne Care to charge your card on file 100% of the Pre-discounted cost for any missed or late canceled appointments. This policy also applies to future appointments.
Due to the nature of the environment, children under the age of 12 are not allowed to accompany you to your appointment.
Diverse Skin Solutions and Acne Care does not allow household pets on the premises with the sole exception of authorized service animals.
SERVICE ANIMALS AND SELECT WORKING ANIMALS INCLUDE:
Guide animals used to guide or provide assistance to persons with disabilities in the activities of independent living. The Americans with Disabilities Act (ADA) defines service animals as any animal individually trained to do work or perform tasks for the benefit of an individual with a disability. If an animal meets this definition, it is considered a service animal.
Leaving an animal in an unattended car is neither appropriate nor safe and is prohibited in some jurisdictions. An animal left in an automobile will be reported to the authorities.
A contraindication is something that would make the procedure unsafe or inadvisable for you. Some contraindications are:
1. You’re using Isotretinoin (AKA Accutane) Wait 6-12 months after stopping to receive Microderm.
2. You have moderate to severe inflammatory acne (schedule the 90-minute Acne/Consultation treatment instead).
3. You have Rosacea
4. You’re In the middle of a cold sore breakout
5. You Have a Rash, sunburn, Wound, or Other Skin Irritation
6. You’re using Topical Retinoids such as Retin A, Differin, Tazorac etc. (wait 2 weeks after using these topicals before receiving microdermabrasion.
7. If you are currently Pregnant
8. If you have metal implants such as pins etc. (dental implants are safe)
9. If you have heart problems or have a pacemaker.
If you have any of these please do not book this appointment. If you do, your appointment will be canceled and your card on file will be charged the full cost of the scheduled appointment.
